14. Behandeling van parodontitis apicalis: desinfectie van het wortelkanaalstelsel

Samenvatting

Wanneer een element wordt geassocieerd met parodontitis apicalis, dan is er sprake van biofilmvorming in en om het wortelkanaalstelsel tot en met het apicale foramen. Het immuunsysteem reageert hierop met een ontstekingsreactie. Volledige verwijdering van de biofilm en micro-organismen is niet mogelijk vanwege het recalcitrante gedrag van de biofilm en de complexe wortelkanaalanatomie. Daarom is het doel van de behandeling van parodontitis apicalis om zo veel mogelijk micro-organismen en biofilm uit het wortelkanaalstelsel te verwijderen. Hierdoor kan, in principe, de reactie van het immuunsysteem op de micro-organismen geblokkeerd worden en genezing van de parodontitis apicalis optreden. De verschillende desinfectieprocedures worden beschreven aan de hand van drie hoofdgroepen: mechanische, chemische en medicamenteuze desinfectie.
