Samenvatting

Janssens HJEM. Jichtartritis. Huisarts Wet 2011;54(5):254–8. Jichtartritis is een aandoening die vooral in de huisartsenpraktijk veel voorkomt: 90% van alle patiënten wordt daar behandeld en slechts 10% bij de reumatoloog. Het is daarom verwonderlijk dat er binnen de huisartsgeneeskunde zo weinig onderzoek naar jichtartritis gedaan is. Een aantal recente onderzoeken in de eerste lijn hebben echter nieuwe inzichten opgeleverd over de diagnostiek, behandeling en cardiovasculaire prognose van dit oude ziektebeeld.
De diagnose jichtartritis is niet goed te stellen als men alleen afgaat op ‘kenmerkende’ klinische symptomen. Niet alleen de huisarts die dat probeert schiet nogal eens mis, maar ook de reumatoloog. Als opmaat naar de gouden standaard, het aantreffen van urinezuurkristallen in de gewrichtsvloeistof, beschikt de huisarts sinds kort over enige extra hulpmiddelen in de spreekkamer: een diagnostische beslisregel en een online jichtcalculator.
Jichtartritis blijkt vaak gepaard te gaan met cardiovasculaire en renale risico's. Jichtpatiënten zijn bijna altijd van gevorderde leeftijd, meer dan de helft heeft hypertensie en bijna eenderde heeft een cardiovasculaire aandoening. Daarom is bij jichtpijn een korte kuur prednison (30-50 mg eenmaal daags) te prefereren boven de gebruikelijke colchicine en NSAID's. Daarom ook moet men bij de diagnose jichtartritis altijd het cardiovasculair risico van de patiënt in kaart brengen. Wanneer hierbij diuretica in het spel zijn, hoeven deze niet vermeden te worden.
