18. Apicale microchirurgie

Samenvatting

Bij endodontisch behandelde gebitselementen met een persisterende periapicale ontsteking kan apicale chirurgie geïndiceerd zijn om de ontsteking te laten genezen. Hiervoor moet met behulp van de anamnese, mond- en röntgenonderzoek de meest waarschijnlijke oorzaak bepaald worden. Als een herbehandeling risicovol is of hiermee de oorzaak niet voldoende kan worden weggenomen, kan een chirurgische benadering een goede behandeloptie zijn. De instrumenten en vulmaterialen die in de endodontische chirurgie worden gebruikt, zijn de afgelopen jaren sterk verbeterd. Gebruik van een operatiemicroscoop, micro-instrumentarium, microhechtmaterialen en de introductie van biokeramische vulmaterialen blijken een positieve invloed te hebben op de prognose van de behandeling. Door zorgvuldige omgang met de zachte weefsels kan de behandeling worden uitgevoerd zonder littekenvorming of recessie. Apicale chirurgie is, na een zorgvuldige indicatiestelling, een waardevolle behandeloptie met een voorspelbare prognose.
