21. Restauratie na endodontische behandeling

Samenvatting

Weefselbehoud en een adhesieve opbouw vormen de beste optie voor elementen met een wortelkanaalbehandeling. Getracht moet worden zo veel mogelijk supragingivaal tandweefsel te handhaven. Op die manier kunnen extra handelingen, kosten en risico’s bij de plaatsing van een wortelkanaalstift veelal achterwege blijven. Als een wortelkanaalstift onvermijdelijk is, moet men zich realiseren dat, ondanks het gebruik van adhesieve technieken, onverminderd van kracht blijft dat moet worden voorkomen dat de retentie van een opbouw louter afhankelijk is van de retentie van de wortelkanaalstift. Het behoud van supragingivaal tandweefsel en het zo veel mogelijk handhaven van ondersnijdingen in de pulpakamer vormen daarbij een belangrijke schakel. Restauraties van composiet, met en zonder overkapping, ofwel indirecte partiële overkappingrestauraties passen het best bij dit uitgangspunt.

In de Engelse taal wordt met de term ‘fer(r)ule’ het metalen buisje aangeduid dat bij gereedschap het splijten van een houten handvat moet voorkomen. Het Nederlands heeft geen woord met precies deze betekenis.
